A good web design questionnaire template saves you an hour on every project. The client answers once, in writing, and you get everything you need to quote, plan and design without chasing details by email. Below is a complete template you can copy for free, plus a short version to send before you quote.
How to use this template
- Before the quote: send only the 8 questions of the short version (end of this article).
- After the deposit: send the full template. The client has committed and answers properly.
- Review it on a 20-minute call: go through blanks and vague answers together.
- Save the answers next to the lead so you and your team find them in seconds.

The short version (send before you quote)
- What does your business do, and where?
- What should visitors do on the website?
- Which pages do you need?
- Do you need booking, payments or a shop?
- Do you have a logo, text and photos ready?
- Two websites you like?
- Budget range?
- Ideal launch date?
These eight answers are enough to write a solid proposal. Use our website design proposal template to turn them into three clear options.
Make the questionnaire easier to answer
- Show a preview first. Clients answer faster when they react to a real page. Generate a preview of their new site from their current one, then use the questionnaire to refine it.
- Prefill what you know. Business name, phone, address and services are often on their current site or Google profile.
- Use multiple choice for budget, timeline and features; keep open questions for goals and customers.
- Allow โI don't knowโ. A blank answer tells you where the client needs your advice.

Frequently asked questions
Is this web design questionnaire template free to use?
Yes. Copy the questions into your own form, document or PDF and adapt them to your projects, with or without credit.
Should I send a PDF or an online form?
An online form gets more complete answers because clients can fill it on their phone and you receive the answers in one place. Keep a PDF for clients who prefer to print and write.
How many questions should a web design questionnaire have?
About 8 before the quote and 25 to 35 once the project is confirmed. More than that and clients stop halfway.
Can I use it for website requirements too?
Yes. Sections 6 and 7 cover features, integrations, budget and timeline, which are the core of a website requirements questionnaire.
